Menstrual Disorders

Menstrual disorders (Abnormal Uterine Bleeding) are a class of problems affecting a woman’s monthly menstrual cycle. Menstrual disorders include any or more than one of the following:

• Menorrhagia (heavy periods)
• Intermenstrual bleeding (bleeding between periods)
• Post-menstrual spotting (bleeding after periods)
• Postmenopausal bleeding (vaginal bleeding after menopause)

To evaluate the problem, a detailed history and full medical examination will be performed. The cause for menstrual disorders can be structural (e.g. fibroids) or hormonal. Following tests and examinations may be performed to find out the cause:

• Endometrial biopsy. A scraping method is used to remove some tissue from the lining of your uterus, to identify any possible problem, including cancer.
• Ultrasound. This is a non-invasive method to check for any structural abnormalities such as fibroids, endometrial polyps or ovarian cysts.
• Hysteroscopy. In this procedure, a miniature telescope-like instrument called a hysteroscope will be used to look into your uterine cavity to check for any abnormalities inside the uterine cavity.
• Dilation and curettage (D&C). During a D&C, your cervix is dilated and instruments are used to obtain tissue from your uterine lining. It may also be used as a treatment for excessive bleeding.

Treatment for menstrual disorders depends on the diagnosis. It includes medications and surgery. Medications include symptomatic treatment to control the amount of bleeding and also hormonal treatment. Surgical treatment will depend on the diagnosis and also considerations, e.g. your fertility wish.
